Appealing: The Format

td;lr: the format ban/mute appeals go into.
This is the format you NEED to put Ban/Mute appeals in, otherwise they will be automatically denied.
The format needs to be:
Type Of Ban (if you got banned):
IGN (In-Game Name)
Reason:
Explanation:
And a note saying that if you break the rules again, any appeal will be denied.
Also, PLEASE request a thread lock.
